Drug adherence in heart failure

Investigators may have uncovered the factors contributing to the initiation of sodium-glucose cotransporter-2 inhibitors in patients with heart failure.

In a retrospective study published in Clinical Research in Cardiology, the investigators used 2022 to 2023 German health insurance claims data and information from the INKAR database to examine the relationship between socioeconomic factors and SGLT2 inhibitor initiation in a group of nearly 70,000 patients with heart failure.

The investigators found that newly coded heart failure, recent enrollment in a coronary artery disease management program and residence in a higher-income tax municipality correlated with a higher likelihood of initiating SGLT2 inhibitors. However, female patients, older patients, those who had pensions or family insurance, and those who moved were less likely to initiate the treatment.

The findings potentially linked certain demographic and socioeconomic disparities to a lower likelihood of guideline-recommended SGLT2 inhibitor initiation for heart failure. The investigators emphasized the critical need for targeted interventions to improve the rate of treatment uptake and care inequities in this patient population.
